Thank you for your interest in participating in the Walk for Life Info Faire. Registration is $75.00 per table and must be received, with payment, no later than January 6, 2015.

Please note: registrations will be accepted on a first-come, first-served basis.  Due to space constraints at Civic Center Plaza, each group will be limited to one table for the time being.  If you wish to add a second table, please check the box at the bottom of the registration form below and your group will be added to the waiting list.  Once capacity is reached, registration will be closed.

Please complete the online registration form below. We recommend printing this form upon completion for your records. After submitting your registration, you will receive a confirmation email containing PayPal links to pay for your table(s). If you prefer to make payment by check, print this confirmation email and mail it in with your check.

Each registration comes with one 6 foot table and two chairs. Groups are not permitted to bring their own tables or racks – only the tables provided by Walk for Life may be used. Your table(s) will not be held for your group unless we have received your reservation form and payment by January 6, 2015. For more information, please email us.
